Child with Medical Needs Binder, Special Needs Child, Hospital Binder I am a mom of an adult child with muscular dystrophy and I am also a retired school administrator and educator. This digital download includes medical forms that can help you organize your child's medical visits and hospital stays. Some of the forms and advice sections were created specifically based on my years of experience. Included in this download is a special note about me and some of the things I have experienced as a parent. I also include hospital advice for parents. Things I wish I had known to make hospital stays with my child better. I also included a hospital packing list for parents and children because this was a lifesaver for me. Some of the forms included in the packet are editable on CANVA so you can customize your child's name in the binder if you wish to. Or you can leave it as a generic medical binder. I hope that the forms and advice will help you stay organized and sane if you are facing a new medical diagnosis of your child. You are not alone. I have been there and am currently still there.
